This is a service work order worksheet that prices the call-out from your figures: a callout or diagnostic fee, miles travelled at your rate per mile, labour hours on site at your rate, parts at cost with your markup, and sales tax on the marked-up parts into a total, plus the figure most call-out businesses never see, the revenue for each hour the technician was on site. The callout and the travel, the lines a shop job does not have
A $75 callout fee and 18 miles at $0.70, or $12.60 of travel, open the sheet. Both are yours: set the mileage rate to zero and put a flat trip charge in the callout line if that is how you bill. The point is that the drive is on the sheet as its own figure rather than absorbed into the labour and forgotten.
Labour on site and parts marked up, with the tax on parts
1.5 hours at $95 is $142.50 of labour, and $62 of parts marked up 25% bills at $77.50 with $4.65 of tax at 6% on that line. Labour is left untaxed, the common case; whether it is taxable in your state is a question for your accountant, and the sheet shows the labour as its own line so the figure is to hand.
The total, and the revenue for each hour on site
The worked service order is $312.25, and divided by the 1.5 hours on site that is $208.17 an hour. The second figure is the one to price the next call from: a long drive with a waived callout fee shows itself as a low number here, on the sheet, where the decision belongs.

What is a service work order?
A work order for a call-out: the order a repair or maintenance business raises when it is sent to a customer's site, with a callout fee and travel on top of labour and parts. Some trades call it the service ticket; the worksheet prices it whatever the name, and the paid record keeps it with a status like any other order.
What is a good revenue per hour on site?
Whatever covers your technician's cost, the truck, the drive and your margin, which is your figure and not this site's. The worked example reads $208.17 to show the arithmetic. The sheet exists so that the number is seen before the next call is priced, not after the month is closed.
