Daily reports, incident reports, inspection sign-offs, delivery notes and toolbox talk registers are records: somebody may need them. Checklists that exist to prompt a crew are habits and can stay on paper. Digitizing everything indiscriminately is how a site ends up ignoring all of it.

What should an incident report contain? Who, what, where and when, the injury or near miss described plainly, the witnesses, what was done immediately, and what has been done since. Write it on the day. An incident report sample is useful for the field list, but the value is the contemporaneous record, not the format.
How long do site records need to be kept? Longer than the job, and how much longer depends on the record and the jurisdiction. Treat anything about injury, inspection or instruction as something you may need years later, which is an argument for a searchable record rather than a box of paper.
Does a field service report form need a signature? A customer signature on site is useful evidence that the work was seen and accepted, particularly for extra work. It is not a legal requirement everywhere, and a dated photograph plus a written entry is better than a scrawl on a pad that nobody can read.
