Blank risk assessment form: what the risk assessment forms a site actually keeps have on them
- Work order total
- $863.16
- Labour
- $510
- Parts, marked up
- $288
Every figure on this page is computed from the inputs entered, by the method stated below it. Jobcardo publishes no tax rate, no wage figure and no trade standard: the rate, the markup and the tax on the parts line are yours, and the defaults are a worked example to replace with your own figures.
A blank risk assessment form is worth having on the van and worth keeping to one side of paper. The fields that earn their place are the job and site, the date and the assessor, the hazard rows with their controls and further actions, the residual judgement, and a line for who has been briefed. Anything else is usually there to make the form look thorough, and every extra field is one more thing left blank at five in the afternoon on a cold site.

Keep the grid to the columns that change behaviour
Hazard, who is harmed and how, existing controls, further action, owner, date. Risk assessment forms that add scoring matrices, likelihood tables and signature blocks for four roles get abandoned on site, and an abandoned form is worse than a short one.
Prefill everything the job already knows
Job number, site, date, assessor and the trade's usual hazards are all known before anybody picks up a pen. A form that arrives with those filled in is a form that gets completed, and the fields that matter are the ones only the person on site can answer.
Get it back off the van the same day
A completed assessment that lives in a folder in a vehicle is not a record. Photographed or filed against the job on the day, it is searchable when a client, an insurer or an inspector asks, which is the only reason anybody keeps it.
Blank risk assessment form: common questions
Does the form have to be on paper? No. Paper is convenient on site and terrible as an archive. The practical pattern for a small firm is to complete on whatever works in the moment and to get the result attached to the job record the same day.
Do I need a separate form for each job? Yes for the issued assessment, because the site changes even where the trade does not. Keeping one master per type of work as a starting point is sensible; issuing the same undated sheet to every job is not an assessment at all.
What makes a risk assessment form fail a review? No date or assessor, controls written as slogans, a further action column with no owner, and no evidence that anybody was briefed. Those four are what a reviewer checks first because they are the four that show whether the document was used.
