As-built drawings record what was actually built, as against what was drawn: the pipe that moved a metre to clear a beam, the duct route that changed, the circuit that was split. They are produced by marking up the construction drawings as the work proceeds and issuing the marked set at handover. Almost every job produces them badly, because the marking up is done at the end from memory, and the resulting drawings are worth very little to the owner who will pay somebody to survey the building again in five years.

Mark up as you go, on the current revision
The only reliable as-built record is one made within days of the work. Mark the change on the current sheet, date it, note who made it and why, and photograph the condition before it is covered. Everything else is reconstruction.
Photograph what will be hidden
Services in walls, floors and ceilings are the whole point of an as-built record. A dated photograph with a tape measure or a grid reference in shot is worth more than a neat drawing produced later, and it is the thing the owner and the next contractor will actually use.
Hand over a set somebody can use
At handover the owner should receive the drawing set marked with the deviations, the photographs indexed to locations, and the operating and maintenance information. As built drawing software helps by keeping the mark-ups against sheets and revisions; what it cannot do is make a record that was never taken on site.
As-built drawing software: common questions
Who is responsible for as-built drawings? Usually the contractor, as a contract deliverable, sometimes with the design team producing the final set from your mark-ups. Either way the raw record is yours, which is why it has to be captured while the work is open.
Are as-builts the same as record drawings? Close enough for most jobs. Record drawings sometimes means the designer's tidied version of the contractor's marked-up as-builts. The substance is the same: what is actually there, as against what was drawn.
What happens if the as-builts are wrong? The cost lands years later on somebody drilling into a service or pricing an alteration from a drawing that does not match the building. On your side it can mean a retention held at handover, because an incomplete as-built set is one of the easiest deliverables for a client to point at.
